Surgical Retractors: Types, Uses and Why They Matter
Retractors hold back skin, muscle and organs so the surgeon can see and reach the operative area. They reduce the need for assistants to hold tissue and improve exposure, which shortens surgery and improves safety.
Hand-held retractors Senn and Army-Navy retractors for superficial and small incisions; Langenbeck and Richardson for abdominal and deeper work; Deaver for deep cavities; Volkmann rake retractors for skin and subcutaneous tissue. They are held by an assistant and can be repositioned quickly.
Self-retaining retractors Weitlaner and Gelpi hold wounds open with a ratchet, leaving the assistant free; Balfour and similar abdominal types provide wide exposure for laparotomy; mastoid and cerebellar types are used in ENT and neurosurgery.
Uses Abdominal, orthopaedic, gynaecological, thoracic and ENT surgery, as well as minor procedures where a clear field is needed.
Quality points to check Smooth edges and rounded blade tips so that tissue is not cut or crushed; solid welds or one-piece construction; ratchets that hold under spring load; no bending under reasonable force; a uniform finish that does not trap debris.
Care, cleaning and sterilization (IFU summary) Always follow the Instructions For Use (IFU) supplied with the instrument and your hospital's sterile-processing protocol. In general: rinse off blood and tissue promptly, never let them dry; open all box-locks and ratchets; clean with a neutral or enzymatic detergent and a soft brush (no steel wool or chlorine bleach); rinse with demineralized water; dry completely; lubricate hinges with an instrument-grade milk; then sterilize by a validated steam (autoclave) cycle. Inspect before every use and remove any instrument that is bent, cracked, stained, or has misaligned tips.
